REFERRALS - Hospice Wairarapa

Here are the details to submit a referral for someone, who requires our support, within the Wairarapa region.

Please complete our REFERRAL FORM HERE via PalCare and press save when complete.

Hospice Wairarapa provides psychosocial & spiritual care for palliative care patients and their Whānau.

IMPORTANT - Medical Referrals - if the person you are referring requires palliative nursing involvement from Kahukura, please make contact with their GP or healthcare provider.

People may self-refer, though we would request permission to confirm diagnosis with your GP or specialist.
