Integrated supportive care services

The Hospice Wairarapa team work alongside other specialist supportive care organisations as part of a network to provide palliative care for those with life limiting illness in Wairarapa.

The network is called The Wairarapa Palliative Care Service and includes Kahukura, the specialist palliative care nursing service in Wairarapa along with FOCUS, the Needs Assessment and Service Coordination Agency. Each organisation, including Hospice Wairarapa, provides different specialist care services in the community. The network aims to provide quality, coordinated palliative care and supportive care services across the region.

The Hospice Wairarapa team also work with local organisations in the community such as the Cancer Society, Age Concern and others who provide services to people in Wairarapa who have life limiting illness.

Kahukura Palliative Care Team

Kahukura is a team of health professionals experienced and trained in palliative care. Kahukura Palliative Care provides nursing advice and care 24 hours a day, seven days a week with support from the 111 Ambulance emergency services.

Kahukura works closely with your GP. While specialist consultant nurses have the primary day-to-day oversight of care, they also support others in the team to provide care, for example through working with nurses providing palliative care in residential care facilities.

The team of people involved in palliative care includes your GP, palliative district nurse, clinical nurse specialist, social worker; and may include a palliative specialist doctor, hospital physician, residential care nurse, occupational therapist, physiotherapist and spiritual advisor.

The Hospice Wairarapa team works with Kahukura as part of the Wairarapa’s palliative care network to provide supportive care services for clients, carers and whānau in the Wairarapa community.

Focus - Needs Assessment and Coordination Agency.

FOCUS is a service of Te Whatu Ora - Health New Zealand, and provides a single point of entry for referrals for palliative care support including Kahukura clinical specialist care. The FOCUS team arrange services to support people to be as independent as possible.

Hospice Wairarapa takes referrals from FOCUS to provide supportive services for clients, carers and whānau.
